1 - application form

2 - the original extract from the commercial or business register or its notarized copy (can be provided by our company)

3 - the decision to assign the DIČ. If an individual entrepreneur has been assigned a so-called "Special Payer Number", also provide a decision on its assignment

4 - attorney

After processing the received information, the authorized customs service employee will hand over the decision on registration in the EORI Commercial Register to the representative of the subject under the receipt, or send this decision by mail.
